Anticoagulation Management Clinic
Individuals who are taking anticoagulation medication (warfarin, Coumadin®) will benefit from our Anticoagulation Clinic. Services are provided by pharmacists with special training in anticoagulation management to outpatients who have been referred by their physician.
Patients must be ambulatory and able to come on-site for an appointment with an anticoagulation clinic pharmacist. Pharmacists see the patients at the clinic located with the Outpatient Pharmacy on the first floor of Ephraim McDowell Regional Medical Center, just inside the Third Street entrance. Cost savings: Patients are eligible for discounted warfarin prescriptions through the Outpatient Pharmacy.
Safer results: We offer point of care testing (INR) during the visit and adjust the dose to reach the targeted INR. Participating in the Anticoagulation Clinic also offers continuity of care for patients to meet National Patient Safety Goals for Anticoagulation Safety.
Education: We educate the patient and help track individual patient variations in diet, drug therapy and goal while monitoring for safety. Referring physicians receive a report after each visit. Pharmacists denote INR and dose adjustments in Meditech. |
